Included in this download are all the forms you will need to register for an event. . . the Individual, Master, Housing and Liability.
Distribute to the participants at your church.
Collect in time to honor the deadline.
Send in:
1. ONE check (church issued) for the entire group for each individuals' deposit which is $75 per participant\.
2. Completed MASTER FORM
3. Completed HOUSING REQUEST form
4. COPIES of each individual registration form and liability waiver form. The ADULT LEADER KEEPS THE ORIGINALS.
